Permission To Skate Forms - Available - UPDATED
To obtain a Permission To Skate Form please complete our online request form. Permission to Skate Forms will be completed within
2 days of your submission and must be picked up at the GCMHA Office upstairs at Bill Burgoyne Arena during business hours of Tuesdays & Thursdays 6:00 pm to 9:00 pm and Saturdays 9:00 am to 1:00 pm (Closed on Saturdays of long weekends) entrance from back parking lot. Parent must sign at time of pick up.You will receive an email confirmation that your form has been submitted. Permission to Skate Forms will NOT be emailed. Please view
