By Signing this form, parents and players have read, understood and agree to the following:
1: In consideration on being permitted to try out, or on being accepted by any of the member partners of the Garden City Minor Hockey Association, I hearby release the Association, the Officials of its members from all claims or causes of action which at any time hereafter may arise whether due to injuries, fatal or otherwise, or loss of, or damage to, personal property.
2: Upon signature of this agreement, the player and parent(s) or representative of the registered player hereby agrees to abide by the GCMHA Constitution, By-Laws and Playing Rules and by the constitutions, by-laws, playing rules and codes of conduct of each of the GCMHA member partners.
3: I acknowledge that I can tryout for GCMHA AA Tryouts only at the cost of $ 100 cash or etransfer to
[email protected] Put child's name and age division in notes on the etransfer.
4. I acknowledge that my child cannot participate in the AA tryout unless I hand in a NRP form at the first tryout. (NRP forms are to be obtained from your home centre).
