First step is to make sure you have notified our Ice Convenor of the date of your tournament / exhibition game. No travel permits will be approved until the Ice Convenor has added them to calendar on our website.
OMHA Approved Travel Permit will be emailed to the email address submitted on this form.
If you are applying for a Tournament Travel Permit you MUST include the Sanction Permit Number. Travel Permits can take 10 to 14 days to be approved.
