I, the undersigned acknowledge that I have never been charged or convicted in a criminal matter in or outside of Canada. I will accept any judgement of the Garden City Minor Hockey Association for my failure to abide by Garden City Policies and Code of Conduct, (both are available on the Garden City website: https://gcmha.ca/Pages/1251/Constitution_and_Policies/ ) which may include suspension or removal from the Coach position. I acknowledge that I may be removed from any coaching position assigned by Garden City Coaches Selection Committee at any time at the sole discretion of the Garden City Executive. I agree to indemnify and hold harmless the Garden City for my failure to abide by Garden City, OMHA, OHF, and /or Hockey Canada guidelines. I support the Garden City philosophy of player development and will promote the playing of hockey. I will accept full rsponsibility from my actions as a team official of the Garden City Minor Hockey Association.
