2023-2024 Permission to Skate Forms will be emailed to the email address you provide on the Online Request Form.

Only players with a St. Catharines address on the HCR, and are in good standing will receive a Permission to Skate Form. 

If you have just recently moved into St. Catharines from another OMHA Centre please contact [email protected] regarding OMHA Residential Transfer paperwork. 

If you have just recently moved to St. Catharines from an Alliance / GTHL Centre please contact them regarding OHF Residential Transfer paperwork. 

"Permission to Skate Forms are required when a player will be attending evaluations/tryouts for their Home Zone.
If the player played outside their Home Zone in the previous season and will be returning to that team, this is not required.
Players’ Home Associations are to issue Permission to Skate Forms applicable only to the AAA Zone for which they are a feeder. Similarly, AAA Zones require Permission to Skate Forms only from their respective feeder Associations.
If a player is returning from the previous season, nothing is required from their Home Association. In the case of players coming from outside a Zone’s respective feeder Associations, a Waiver (possibly multiple Waivers) is required."
