Players Birth Certificate Verification & Rowan's Law Acknowledgement Foms

BIRTH CERTIFICATES - I will be at the Sportspad on the following dates and times to view players birth certificates:
Wednesday August 21st from 4:30 pm to 8:00 pm @ SP1
Monday August 26th from 5:00 pm to 8:00 pm @ SP4
Wednesday August 28th from 6:00 pm to 8:00 pm @ SP1  
Parents can also drop by the GCMHA Office on Tuesdays & Thursdays from 6:00 pm to 9:00 pm or Saturdays 9:00 am to 1:00 pm with their child's birth certificate. If you are meeting with your parents prior to August 21st and would like me to attend the beginning of your meeting to view the Birth Certificates then please let me know the date, time and where. Please remember I will not be able to attend if it is during our office hours. If you would like to book the GCMHA room to have a meeting with your parents and get them to bring the Birth Certificates and sign the Rowan's Law Acknowledgement Form during our office hours please let me know, date and start and end times of meeting.  
ROWAN'S LAW ACKNOWLEDGEMENT FORMS - Manager or Coach must drop the forms off as a package to the GCMHA Office or to me at the Sportspad on the dates listed. PLAYERS FORM - must be completed and have both the child and parents signature on it. COACHING STAFF FORM (INCLUDING ON ICE HELPERS) - must be completed and signed by staff member "ALL FORMS - must have original signatures - I cannot accept forms that have photocopy signatures or forms that have been emailed to you or me."
