Permission To Skate Forms
Our Permission to Skate Request Form is now available. Once completed your PTS form will be available for pick up at the GCMHA Office.
Office hours are Tuesdays & Thursdays 6:00 pm to 9:00 pm and Saturdays 9:00 am to 1:00 pm. Closed March 31st and April 5th.
Parent must sign the Permission to Skate when picking it up.
